It depends which of UnitedHealthcare Community Plan's two Michigan Medicaid plans your loved one has:

  • If they qualify for both Medicare and Medicaid, they may be enrolled in UnitedHealthcare Community Plan's Dual Eligible Special Needs Plan (D-SNP) through MI Coordinated Health — our eligibility check confirms this for you.
